Text
Names, dates, phone numbers, short answers
Set a maximum length for codes such as ZIP codes or IDs.
Paragraph
Comments, addresses, descriptions
Make it tall enough for 3 to 5 lines; text wraps automatically.

Yes/no questions, consent, “check all that apply”
Each checkbox is independent, so several can be ticked.
Radio button
One answer out of a few options
Give every button in a group the same group name and a different value.
Dropdown
One answer out of many options (countries, sizes)
Turn on “Allow typing a custom choice” for an “Other” answer.
Signature
Where the person signs the form
Signing works in Adobe Acrobat and Reader and in many e-signature apps.
Tips for a professional fillable form
Use clear field names. Names such as first_name or email appear when form data is exported to a spreadsheet, so they are worth getting right.
Fit fields to the printed lines. Zoom in and draw each field just above the underline or inside the printed box. Arrow keys move the selected field by 1 point (Shift + arrow: 10 points).
Keep font size on Auto for short fields so long entries shrink to fit, or choose a fixed size (10 to 12 pt) for a uniform look.
Mark required fields so PDF readers can warn people who leave them empty.
Test the result by opening the downloaded file in a PDF reader and pressing Tab to move through the fields.
What gets saved
The tool writes a standard interactive form (AcroForm, defined in ISO 32000) into your PDF. Each field gets a widget annotation with its own appearance, so the form looks the same in every viewer, uses the Helvetica font for entered text, and keeps the original page content untouched. To lock a filled-in form so nobody can change the answers, use Flatten PDF.
Frequently asked questions
How do I make a PDF fillable for free?
Open the PDF here, choose a field type such as Text or Checkbox, and drag a box over each blank on the page. Name the fields, then press Create fillable PDF. The downloaded file contains real form fields that anyone can fill in with Adobe Acrobat Reader, a web browser or a phone PDF app.
Which field types can I add?
Single-line text fields, multi-line paragraph fields, checkboxes, radio button groups, dropdown lists and signature boxes. Text fields can have default text, a font size, alignment and a maximum number of characters; dropdowns can allow custom entries.
How do radio buttons work?
Radio buttons with the same group name belong together, so choosing one clears the others. Each button in a group needs a different option value, such as Yes and No. Clicking with the Radio tool after selecting a radio button adds another choice to the same group.
Can people sign in the signature box?
The signature box is a standard PDF signature field. Adobe Acrobat and Reader show a sign-here area where users can add a digital signature or a drawn signature. Some simple viewers display the box but do not support signing.
Will the form work in Chrome, Preview and on phones?
Yes. The fields are saved as a standard AcroForm with appearance streams, which Adobe Acrobat Reader, Chrome, Edge, Firefox, macOS Preview and most mobile PDF apps can fill in and save.
Can I edit a PDF that already has form fields?
Yes. Existing fields are kept unchanged and your new fields are added next to them. New field names must be different from the existing ones, and the tool warns you if a name is already taken.
Is my PDF uploaded to a server?
No. The PDF is rendered and the form is built in your browser with JavaScript. Nothing is uploaded.
