How to use the Pet Vaccination Record Maker
Fill in the pet details and, if you like, add a photo. The sample pet shows how a finished record looks.
Enter each vaccine with the date given from your vet's certificate. The next due date fills in automatically; change it to match your vet's paperwork.
Add other care such as heartworm tests, flea and tick prevention or a wellness exam.
Check the reminders: overdue items are red and items due within 30 days are amber.
Download the PDF or print the record, cut out the wallet cards and import the .ics file into your calendar.
Typical vaccine and care intervals
These are the default intervals the tool uses to suggest the next due date for adult pets. They follow common veterinary guidance, but your veterinarian's schedule, the specific product and local rabies law always come first.

Puppies and kittens get a series of core vaccines every few weeks until about 16 weeks of age. See the puppy vaccination schedule for that first year.
Tips for keeping a pet health record
Copy dates from the certificate, not from memory. The certificate also lists the product, lot number and expiry; the Notes column is a good place for the lot number.
Record the microchip number and make sure it is registered with your current phone number.
Update the record after every vet visit and print a fresh copy for the pet sitter, kennel or travel folder.
Bring the record to a new vet together with the original certificates so they can plan boosters correctly.
Frequently asked questions
Is this an official rabies certificate?
No. It is an owner-kept summary. A rabies certificate must be issued and signed by a licensed veterinarian, and boarding kennels, groomers or authorities may ask to see that original. Keep the vet's certificates and use this record as a handy overview.
How is the next due date calculated?
When you enter the date given, the tool adds a typical adult booster interval: 1 or 3 years for rabies depending on the product, 3 years for DHPP and FVRCP after the first-year booster, and 1 year for leptospirosis, Bordetella, canine influenza, Lyme and FeLV. Puppies and kittens need a series of shots a few weeks apart, and local rabies laws vary, so always use the date your veterinarian gives you.
Which vaccines are core for dogs and cats?
For dogs, rabies and the distemper, adenovirus (hepatitis) and parvovirus combination (often given as DHPP or DAPP) are considered core. For cats, rabies and FVRCP (panleukopenia, herpesvirus and calicivirus) are core, and FeLV is recommended for kittens and for cats at risk. Others such as Bordetella, leptospirosis, Lyme or canine influenza depend on lifestyle and region.
What is on the wallet cards?
The first card shows species and breed, birth date, microchip number, the latest rabies date and due date, and owner and clinic phone numbers. The second card lists the next due dates. Each card is 3.5 by 2 inches, the size of a business card.
How do the calendar reminders work?
Download reminders (.ics) creates one all-day calendar event for each future due date, with an alert 7 days before. Open the file with Google Calendar, Apple Calendar or Outlook to add them.
Is my pet's information private?
Yes. Everything is processed in your browser. The record and photo are saved only in this browser's local storage so they are still there next time, and you can erase them with Clear form.
Can I keep records for more than one pet?
The form holds one pet at a time. Download a PDF for each pet, then use Clear form (or simply edit the details) to make the next record.
