Shaded boxes become typeable form fields when “Fillable PDF” is on; they still print as clean boxes for handwriting. Check your state and local rules before adding questions about age, salary history or criminal records.

Questions to handle with care
A job application should only ask for information you need to judge whether someone can do the job. In the US, these topics are commonly restricted or best left off the form:
Avoid asking
Ask instead, if job-related
Date of birth, age, graduation years
Are you at least 18 years of age? (or the minimum age the job requires)
Citizenship or country of birth
Are you legally eligible to work in the United States?
Marital status, children, pregnancy
Can you work the schedule required for this position?
Disabilities, health conditions, medications
Can you perform the essential functions of this job, with or without reasonable accommodation?
Religion, race, national origin, sex
Nothing; these are protected characteristics
Current or past salary (banned in many states and cities)
What pay are you looking for?
Criminal history (restricted on initial applications in many places)
Follow your state and local ban-the-box rules; ask later in the process if allowed
This is general information, not legal advice. Rules vary by state and city, so check with your state labor department or an employment attorney if you are unsure.
Tips for using your application form
Post the fillable PDF on your careers page or attach it to job ads, and keep printed copies at the front desk for walk-in applicants.
Ask every applicant for the same position to complete the same form, which makes comparisons fairer and easier.
Store completed applications securely, since they contain personal contact details, and keep them for at least the period your record-keeping rules require.
Remember that the application does not replace Form I-9, the federal employment eligibility verification form completed after you hire someone.
Frequently asked questions
What should an employment application include?
A standard job application asks for the applicant's contact details, the position applied for and availability, work eligibility, education, recent work history with dates and duties, references, and a signed statement that the information is true and that the employer may verify it. This generator includes all of these, and you can turn optional sections off.
How does the fillable PDF work?
When Fillable PDF is on, every shaded box becomes a PDF text field and every square becomes a checkbox. Applicants can open the file in a free PDF reader or most browsers, type their answers, tick boxes and save or print it. The same file also prints cleanly for anyone who prefers to fill it in by hand.
What questions should I avoid on a job application?
In the US, federal equal employment laws mean you should not ask about race, color, religion, national origin, sex, pregnancy, disability, genetic information or age beyond whether someone meets a minimum age. Many states and cities also restrict asking about salary history or criminal records on the initial application (often called ban-the-box laws). Ask whether the applicant is authorized to work in the US rather than about citizenship, and check your local rules.
Is it legal to ask if an applicant is at least 18?
Generally yes. Asking whether an applicant meets a minimum age, for example for child labor rules or jobs that require it, is common. Avoid asking for a date of birth or exact age on the application; if you do not need the question, untick it.
Can applicants sign the PDF electronically?
The signature box is a regular text field, so applicants can type their name in it, or print the form and sign by hand. If you need a legally verified electronic signature, use a dedicated e-signature service with the completed PDF.
How long should employers keep job applications?
Under US EEOC record-keeping rules, private employers generally must keep employment applications and other hiring records for at least one year from the date the record was made or the hiring decision, whichever is later. Some laws and federal contractors require longer, so check what applies to you.
Is my company information uploaded?
No. The form is generated entirely in your browser, and the PDF is created on your device. Your settings are saved only in this browser so you can come back and make changes later.
